Registered Nurses (RNs) and Licensed Practical Nurses (LPN's) under the direction of the RN, provide skilled nursing care that can only be performed safely and effectively by a nurse. Skilled services include wound care, IV therapy, medication management, pain and symptom management, education on all disease processes and instruction in the care of the patient. The goal of the nurse in home care is to help the patient and/or caregiver to become independent in his or her care. The RN, as case manager, works closely with other members of the team and with the primary physician to provide quality care.
